Community Engagement in Health Care, The Change Foundation, 2012.

This web site provides an array of resources and events produced by The Change Foundation, an Ontario-based health care think tank, intended to generate dialogue and new thinking about health system integration and improvement of the patient care experience. Resources such as online reports, downloadable presentations and video “hot talks” document expert panel discussions, case studies and tools centered around experience-based design in health care. Links are also provided to an interactive site where residents of Ontario can share their own stories about accessing the health care system and to the Partners Advancing Transitions in Health Care (PATH) project, a pilot project bringing together patients, caregivers, and service providers to find new solutions to make it easier for people with multiple chronic conditions to access multiple health care services.
